I have started getting pains in my left knee on descents. It feels like a nasty twist in my knee, but goes away when I stop walking. I wear insoles for fallen arches. Anybody any ideas how to prevent or the cause? Or any ideas on exercises to strengthen the area if it turns out to be a basic weakness? Thanks

We contacted Andrew Stanley, podiatrist at the Rebound Clinic:"Knee pain on descent is the most common walking problem that I treat. It is basically caused by too much internal rotation of the leg/knee when walking. This mis-alignment in the knee is emphasised when coming down hill as the quadriceps (thigh muscles) are doing most of the work, which highlights the dysfunction by inducing stresses that produce symptoms such as pain and swelling. When the work load on the quads is reduced i.e. on the flat, the symptoms are reduced. "Accurately made orthoses (foot correction supports), designed and fitted in the boots by a degree trained podiatrist (BSc Hons PodM) who specialises in biomechanics and walking injuries, plus exercises for the quads usually sort this problem out."We also consulted physiotherapist Dean Payne of Optimum Physiotherapy & Sports Medicine: "Initially I would get this problem correctly diagnosed by a sports physiotherapist before attempting any strengthening exercises as these could do more harm than good depending on the diagnosis. "However if it does appear to be only a weakness then exercises such as single leg squats, lunges and step-ups are great for conditioning walkers but must be done correctly to ensure the desired outcome."

Yep, knees are taking a bashing during most things we do on our feet. I find that the supplement Chondroitin (con droy tin) works wonders. It is made of shark fin (get good quality products e.g. Bio Care) and works to strengthen and rebuild worn cartilage and tendon material. Also, leg raises and leg press exercises on cable machines in the gym work to strengthen the surrounding muscles, ligaments and tendons thus stabilising the knee joint. Couple this with properly fitting boots and the lightest pack you can muster and things can get much more comfortable. Good luck to you. Stig

I also suffer from ITBFS and going downhill without my poles is a definate no no. It has been interesting to read what else other people do for this condition I am at the serious end of this and am currently waiting to have my bands released.....what ever that is sounds nasty to me but my doctor has been great and I have managed to get an appointment very quickly to see the specalist. What do you think of the insoles? I have been tempted to buy some but in these hard economic times am a bit shy of doing so because I don't know if they are just a gimmiky idea to make someone else rich. I have also seen the straps and wondered how they work. can anyone tell me? Cheers Fian
